Creating electronic forms . . . video how-to

When you are ready to go down the Path to Paperless one of the Dentrix modules you will want to become very familiar with is the Questionnaire Module.  This module will turn your ordinary paper forms into electronic forms that can be linked to your website, iPad, tablet or kiosk workstation.  This video will take you though a demonstration on how to create a new form.  Enjoy!

  1. switching over to a electronic forms management system instead of our old paper forms has really created a lot of space, reduced clutter and is so very simple!

  4. Can these forms be emailed to the patient prior to their appointment? How is the questionnaire information automatically updated as a new patient or in an existing chart or health history?

    1. The forms are accessed through a link on your website. When the patient clicks on the link they can fill out the form from home or work and click on Submit. Then the office gets a link that the form has been filled out and they can then link up the form with the patient account. There is a icon called Update Patient Information in the Questionnaire module to update patient information.
